Technically, since The Butcher spawns directly into the player, it shouldn’t have enough time to remove kills from nearby enemies. So the current theory is that The Butcher spawned, but didn’t chase after the group, which means that Avenger kept feeding and feeding him as the group continued to kill nearby enemies.
This can apparently happen with any enemy, as some from the comment note – like Spider Host, for example, which is a different kind of nightmare.
