Blizzard clarifies where and when Diablo 4’s rarest items can drop following player confusion

As the latest in the Diablo series, Diablo 4 loot is supposed to include certain legacy items. Those unique drops are some of the rarest in the game. So rare, in fact, that for a while after launch, no one found them.
This made players think that they might not actually be in the game, a sentiment that didn’t change until someone found one, prompting Blizzard to clarify whether or not said Uniques are in the game and how to obtain them.
The developer confirmed where and when you can expect them to drop, but the odds are extremely rare, so you’ll have to push yourself for a while before you have any chance of seeing them, or get really lucky I guess.
So what are they? According to Diablo 4 Lead Class Designer Adam Jackson, there are currently six Ultra Rare unique drops in the game. Some of their names, like Grandpa, should be familiar to veteran Diablo players.
Jackson took to Twitter to confirm that all six will only start dropping after level 85, but can drop anywhere in the game where standard Uniques can be found. They are extremely powerful: they always drop to 820 power and offer very useful boosts.

They are:
- Doombringer – Unique Sword (Slashing): Lucky Hit: Up to 20% [15-25]% chance to deal 5822 Shadow damage to surrounding enemies and reduce damage dealt by 20% for 5 seconds.
- Grandpa – Unique Two-Handed Sword (Slashing): Increases your critical hit damage by 76% [60-100]%
- Ring of Starless Skies – Unique Ring– Each consecutive primary ability cast reduces the resource cost of your next primary ability by 8% [8-12]%, up to a maximum of 40%.
- Visage of Andariel – Unique Helm: Lucky Hit: Up to 19% [15-20]% chance to unleash a poison nova that deals 10,077 poison damage over 5 seconds to enemies in the area.
- Harlequin Crest – Unique Helm: Gain 13% [10-20]% Damage reduction. Also, gain +4 ranks in all Skills.
- Selig’s Melting Heart – Unique Amulet: Gain +30% Max Resource. Also, when you take damage, it drains 6 [8-3] Resource for every 1% Health you would have lost instead.
As expected, the response from the players has not been very positive. One of the most popular threads on Reddit today is one in which a Reddit user calculated the extremely rare odds of finding them. Most of the answers agree that Blizzard needs to make those items a bit more common so normal players can at least have a chance to get them (beyond being very lucky).
Drop rates are one of those things that can and will be adjusted over the life of the game. Diablo 4’s first major patch is expected to drop before the start of Season 1 sometime in mid-to-late July, and we expect it to include some adjustments to lower rates on those (and other) items.
